Pet's Current Vaccination Record
*We will try our best to honor your requested date and time.
Your email must include a copy of your dog's current vaccination record for rabies, distemper (DHPP), and bordetella. Please note that expiration dates must be present. Without this, we cannot secure your appointment.
All new clients are also required to complete and submit the appropriate form(s) found below. These may be sent via email alongside appointment requests and vaccination records.

Additionally, if your pet has recently visited the doctor and received updated vaccinations, please send us a copy via email/text prior to requesting your appointment.
Please complete this form if you would like to be placed on our waitlist for grooming. If there are any availabilities in our schedule for the day(s) you have selected, we will refer back to this form and be sure to contact you. In addition to completing this form, we encourage you to schedule an appointment for a future date in the case there are no availabilities for waitlist applicants on the day(s) requested.
All clients are required to fill out and sign the appropriate form(s) below prior to your first grooming appointment:
(1) New Client Registration Form (*required)
(2) Matted Release Form (not required for smooth-coated, bath ONLY dogs)
(3) Senior Release Form (only for pets 8+ years)
Please submit a copy of the completed forms via email alongside copies of your dog's current vaccination records.
Please ensure that you have carefully read our store and booking policies, as well as waiver information and vaccination requirements.

As required by NYS Law, all clients must submit copies of their pet's updated vaccination record for the following three vaccines:

We cannot accept pets without proof of documentation that they are current with all three vaccinations listed above.
It is the client's responsibility to submit copies of updated vaccination records whenever necessary prior to scheduling an appointment. In other words, if your pet has recently visited the doctor and received updated vaccinations, please send us a copy ASAP via email/text prior to requesting your appointment. Even if you do not have a grooming appointment scheduled in the near future, we would appreciate them anyway.
New clients, please note that a copy of your pet's current vaccination record of all three vaccines must be submitted via email alongside your appointment request. Otherwise, we will be unable to secure your appointment.
